Clients will use Simple Practice to fill out intake paperwork, contact their provider, view appointments, and pay their balance. Each client must have their own email to submit an intake and create an account, including minors.

We will need your insurance provider and member ID number, reason for seeking therapy, and any provider or scheduling preference you have.
